In the event
you have matters of complaint concerning the professional conduct of a
licensee under the authority of the Louisiana State Board of
Chiropractic Examiners, please use the procedure below.
HOW TO FILE A COMPLAINT: Submit a written or typed, signed statement (narrative form) to the LA Board of Chiropractic Examiners, either by mail or email, providing details of the situation about which you wish to complain and the name of the chiropractic physician involved. Include:
Also attach copies of any materials received from the chiropractic physician that may relate to the complaint being filed. Provide your name, address and a daytime telephone number where you can be reached by the Board, if necessary. You will receive information regarding your complaint as soon as possible.
